Registered Nurse (RN)

Overview
The Adult Services Division provides services to individuals with ID/DD and Autism living in a variety of settings. Each individual is assisted in establishing and maintaining a home that is safe, secure, and comfortable. The program strives to maintain the stability of the person’s home by increasing or decreasing services as appropriate. The RN will be a compassionate, professional member of each individual’s team to provide medical consultation and supports. Duties include accompanying individuals and their staff to medically complex appointments; assess, plan, implement and evaluate individual’s medical care plans; communicate with and/or prepare other staff to communicate with medical providers; interpret and evaluate diagnostic tests to identify and assess individual’s condition; monitor individual’s medical care plan including diet and physical activity; instruct individuals and staff on topics such as vital signs, and health education and disease prevention. This is a 20 hour per week position on site within program operating hours.

Duties

  • · Maintain all certifications required to provide direct services based on program or service model
  • Actively participate in ongoing training, including orientation, mandatory, refresher, and professional development
  • Attend yearly nurse and skills trainer meetings
  • Maintain wellness oversight of individuals with health care needs and chronic conditions
  • Attend Medication Reviews for individuals who are administered medications
  • Oversight of all MAP related matters including quarterly audits on MAR’s and protocols to ensure MAP guidelines are being followed at all times.
  • Assess needs of individuals being referred to the agency and provide feedback to the team.
  •  Provide and maintain documentation of education-based resources shared with individuals encountering new condition, medication, or physician ordered recommendations
  • Provide consultation to staff regarding medical treatments and medication
  • Modify medical treatment plans as indicated by physician recommendations
  • Monitor, record and report symptoms and changes in individuals conditions
  • Document incidents in HCSIS and the EHR system
  • Provide training for staff in vital signs, epi-pen, enema, standard precautions, nutrition and special diets, diabetes, diabetic foot care, fall risks, incontinence, skin care, handwashing, lifting, hearing aids, glucometer use, dehydration, end of life care, infection control, seizures, blood borne pathogens, swallow dysphasia, and other areas as needed and as outlined in the annual training calendar
  • Prepare staff to accompany individuals to medical appointments, accompany as needed
  • Respond to needs for care and provide education to the individuals and their family members
  • Consult and coordinate with healthcare team members to assess, plan, implement and evaluate patient care plans
  • Collaborate with team to ensure individual care plans are being followed
  • Collaborate with individuals teams to determine number of hours needed and responsibilities for PCA’s
  • Other duties as assigned by Supervisor

Qualifications

  • Must possess a current, active license as a registered nurse under the MA Board of Registration in Nursing
  • Experience in the care of individuals with a wide spectrum of diagnoses
  • Familiarity with Electronic Healthcare Record Systems and HCSIS
  • Proficiency in MS Office Suite, Power point, and MS Outlook
  • Strong communication skills, both oral and written
  • Demonstrated ability to efficiently balance concurrent and changing priorities
  • Acceptable driving record, valid driver’s license and access to an automobile suitable to transport individuals
  • Excellent communication skills and the ability to work collaboratively within a healthcare team.

Job Type: Part-time

Pay: $36.00 – $38.00 per hour
