What We’re Looking For:

The ideal candidate for the Outpatient Therapist will be an energetic, upbeat, and highly motivated self-starter who can work well independently and as part of team. Someone who is detail-oriented and processes strong organization, time management, and problem-solving abilities. The ability to exercise good judgment in difficult situations.

The Role:

The Harbor Counseling Center (HCC) is an outpatient mental health clinic dedicated to providing the highest quality mental health services to individuals with cognitive and/or developmental disabilities including autism. The Center’s goal is to help the people it serves reach their highest level of emotional and social functioning, while treating each person with dignity and respect. Complies with all principles of Human Rights, DPPC, Department of Developmental Services , the Division of Medical Assistance, the Department of Public Health, and all other applicable funding regulations as adhered to by Lifeworks.

What You’ll Do:

Outpatient Therapist is responsible for establishing clear treatment goals and clinically appropriate recommendations for treatment and interventions. This role will provide case management and clinical (individuals, family, group) counseling to clients and their families.

Job Responsibilities:

  • Prepare a Treatment plan, with goals written in behavioral terms, within a specified time period;
  • Complete Progress Notes for each therapy session using Harbor Counseling Center’s (HCC) electronic health record;
  • Prepare Annual & B-Annual Reports, termination summaries and all other forms in HCC’s electronic health records;
  • Incorporate feedback from the Utilization Review Team as required;
  • Bring any concerns regarding clinical issues or risk management to the attention of the Director.
The Therapist will keep proper boundaries with the clients and all collateral staff and family members involved with each client. The Therapist will follow HIPAA policies and the Agency’s confidentiality policies as outlined in the Personnel Policies and Procedures received upon hire.

The Therapist will provide the services described above according to a schedule established by the Therapist. These services may be performed at various locations as arranged between the Therapist and their client(s).

  • Provide clinical oversight and leadership to all assigned cases.
  • Complete tasks associated with client intake process.
  • Update treatment plans.
  • Provide individual and family therapy as indicated in the treatment plan.
  • Facilitate groups as needed in support of the Center’s population needs.
  • Complete progress notes in electronic health records.
  • Ensure all contractual obligations are being met by treatment team.
  • Ensure regular communication regarding cases with other team members.
  • Attend supervision as requested by supervisor.
  • Attend regular office meetings.
  • Provide 28 billable units.
Education and Experience
  • A Master’s degree required.
  • Masters of Social Work, Masters in Education (LMHC, and LICSW) are preferred.
  • Experience with electronic health records preferred.
  • Reliable means of transportation
  • Driver’s license in the state of residency
