Job Title: Licensed Practical Nurse or Registered Nurse, Day Hab

Reports To: Health Care Supervisor

Note: This is a 40 hour job. M-Th 8 am – 4 pm, F 7:30 am – 3:30 pm.

SUMMARY
Ensures the health and safety of individuals by assessing individuals for specific nursing needs.
Provide quality training and services that will increase the individuals’ self-sufficiency and independence.
Serve as a role model for others through positive and respectful interactions with clients. Complies with
all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of
Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be
assigned.

Administer and safely store all medical supplies and prescribed medications.

Treat any daily health care problems, including first aid and emergency problems.

Regularly assess clients for specific nursing needs, and/or health problems and monitor accordingly. Lift and
transfer individuals as appropriate to their needs.

Participate in or complete nursing assessments, progress notes, and other applicable documentation.

Assist in coordination of health care services with Speech Therapist, Occupational Therapist and Physical
Therapist.

Provide individual and small group instruction in self-care, activities of daily living, communication,
social and motor skills.

Participate in team meetings regarding general program development, including curriculum development
and schedules.

Comply with all applicable State and Federal regulations, agency policies and procedures and personnel
practices.

Act as resource person to program staff, clients, families and care providers regarding health care issues
and concerns.

Attend training sessions and staff meetings as required.

Protect the health and safety of all consumers and staff.

Perform other duties as requested by supervisors.

This job has no supervisory responsibilities.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
Current license with the Massachusetts Board of Registration in Nursing and one year of experience.

LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and
procedure manuals. Ability to write and proofread routine reports and correspondence. Ability to speak
and present effectively before individuals or employees of an organization.

MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common
fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or
diagram form. Ability to deal with problems involving several concrete variables in standardized
situations.

VEHICLE/TRAVEL
Requires a legally insured and inspected automobile for use to be presented at work and available for use
during working hours. If the program has an agency vehicle, staff are expected to drive it as needed.

CERTIFICATES, LICENSES, REGISTRATIONS
Valid and current driver’s license from the state of residency.
