The role!

Position: Adult Family Care Nurse (RN or LPN)
Location: Wakefield, MA
Pay rate: $35 per hour – Tiered pay rates based on years of service, pending performance evaluations
Hours: Part-time, 12-16 hours per week, Monday to Friday with some flexibility based upon family needs
This position will provide support both remotely and in-person.
Summary & Responsibilities

The AFC Nurse provides nursing case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals.

  • Conducting on-site and telehealth visits with each participant and caregiver in the qualified setting monthly or bi-monthly and as needed.
  • Developing, implementing, and reviewing the AFC plan of care.
  • Training, evaluating, and supervising AFC caregivers in conjunction with the case manager.
  • Completing nursing assessments and progress notes, ensuring the documentation is completed in a timely manner.
  • Submitting a semi-annual health status report to the member’s primary care physician and communicating with primary care physician as needed.
  • Assisting the participant in obtaining information and accessing other healthcare and community services.
  • Interfaces with other agencies to coordinate services.

Qualifications

  • Registered Nurse or Licensed Practical Nurse with current Massachusetts license. New Grads welcome to apply!
  • A demonstrated ability to make appropriate, accurate judgments including handling of emergency situations, and the mental and physical ability to successfully perform job duties and responsibilities.
  • The ability to communicate, monitor and report effectively on an individual’s progress, both verbally and in writing with individuals, families, physicians, service providers and case managers.
  • Demonstrated ability to set limits, to solve problems independently, and to appropriately protect confidential information.
  • A passion and dedication to supporting our people is a must
  • Valid Driver’s license, reliable transportation, and acceptable driving record
  • Must pass company background and reference checks
